The Food Farmacy program is an innovative collaboration aimed at supporting Harris Health patients through essential food assistance, focusing on nutritious items such as fresh fruits, vegetables, and proteins. Patients have the opportunity to sit down with an on-site Community Health Worker (CHW) to help them understand the important connection between diet and health within their daily life. In addition to assisting with program enrollment and navigation, the CHW will provide valuable nutrition education and ensure that every patient receives the comprehensive care they need for a healthier lifestyle.
